I think that if we allow Henry's "Plus the current 92" you've probably got most of them, but to be honest I'm too lazy/knackered to check. Here's the link anyway, but I think there's one or two of The Leagues listings that you can argue with, like how are Wimbledon and AFC Wimbledon counted as the same club?
