    I assumed he managed Palace in the Prem under Mark Goldberg, completely forgot he managed them previously too.

    Jim Smith - Birmingham City, QPR, Newcastle, Derby (think he was sacked from Oxford after he got them promoted to Div One so didn't manage them in top flight)
