    Don't know but plenty of his relatives have officiated at The Valley over the years.

    Clarke (or possibly Gray) - Leeds and Forest (possibly Villa)

    Neal, Thompson and Kennedy ×2 would do this for Liverpool but I think Keegan would achieve it first for Liverpool and Hamburg.

    All were eligible to play for the USA because they were born there?

    Forest aren't in Nottingham, they are in West Bridgford. So if County versus Forest is a derby, so is Forest versus County, meaning West Bridgford also hosts a derby, making that nine towns/cities.

    Mmm... well that casts a doubt over London (no one plays in the city) , as well as the aforementioned Manchester derby, also Stoke as Port Vale play in Burslem...
